Carolina Visions Family Care welcomes referrals from care coordinators, guardians, managed care organizations, hospitals, and community partners across North Carolina.

We understand that making a referral is more than paperwork; it’s about ensuring the right fit, the right support, and the right environment for each individual.

We provide services for individuals with:

• Intellectual and developmental disabilities (IDD)
• Co-occurring mental health diagnoses
• Behavioral support needs
• Medicaid-funded services
• Individuals requiring structured residential supports
